CoolSculpting Treats Pseudogynecomastia


True gynecomastia is male breasts or moobs that are made up of both breast tissue and fat, affecting at least 60% of all males at some point in their life. True gynecomastia must be corrected by excision surgically of the breast tissue and removing the fat by liposuction. There are many surgeons who attempt to correct man boobs by liposuction alone necessitating a “redo” surgery for removal of the breast tissue.

When there is breast tissue in addition to the fat, the breast tissue will not go through the cannula of the liposuction machine, due to the fact it is too dense. This is a very complicated gynecomastia case, in which, most doctors would perform a mastectomy for the removal of the skin, fat and breast tissue. The first stage is done under general anesthesia.  Dr. Delgado will remove the breast gland perform liposuction for both the chest and the axilla region.  A breast lift will be performed with an incision above the areola and a crescent  or half moon shape of skin excised above.  This will lift the skin to further reduce the breast fold. 

This photo is after the first stage in which the breast tissue has been removed and the superior crescent lift  has been performed.  Tacking sutures have been placed  above the areola to help maintain the skin's position.

 You will notice that after the first stage, the mass of breast tissue and fat has been removed but the lax skin remains.  The second stage will be performed after the blood supply has re-established itself  to the nipple-areola area after 4 to 5 months.

The above is the patient laying down and we are looking at his right chest 5 months after the first stage. The outline is the peri-areolar lift with the doughnut shape skin in the center to be excised and the outer circle being closed to the inner circle as a purse-string closure.

The above photos show him at week 4 after the periareolar lift.  The skin puckering will improve over time.

Gynecomastia surgery be performed in many ways.  A variety of factors  go into the technique that a surgeon selects. It may depend on the anatomy of the patient, the severity of the gynecomastia, the patient's desires and the surgeons experience and skill.

A concern that I hear  from men are the change in the nipple region when the arms are raise above the head or the muscle is flexed.  Eventhough it looks great in the normal arms down position, the other postion look somewhat abnormal.  This is due to scar tissue which results from surgery and the removal  of the breast and fat tissue.


The "dynamic approach" will remove the glandular tissue which results in a crater, then the fat tissue on either side is sewn together which closes the space.  This results in a more normal  look when the arms are raised and less of a abnormal look with muscle contraction.  This procedure can be performed through a small incision.  This dynamic technique can not be done for all patients.  The photos below show an intra operative case.

ADOLESCENT GYNECOMASTIA

San Francisco, California - Adolescent gynecomastia occurs during the early teenage years typically 12 to 13 years old.  The hormone balance is very critical during this time.  The hormone estrogen encourages female characteristics to develop and testosterone causes male development.  The imbalance of these hormones with an over balance of estrogen will cause breast bud development in males causing gynecomastia.
